Our esteemed client in the Public Healthcare sector is seeking a passionate and experienced Case Management Officer to join their team. In this role, you will provide comprehensive, patient‑centred care for heart failure patients, working closely with a multidisciplinary team to optimize clinical outcomes, enhance patient quality of life, and ensure continuity of care across the healthcare continuum.
Key Responsibilities:
- Conduct thorough assessments of patients’ medical, psychological, social, and financial needs
- Develop, implement, and monitor individualized care plans in collaboration with doctors, nurses, and allied health professionals
- Track patient progress through clinic visits, telehealth monitoring, and regular follow-ups
- Educate patients and caregivers on heart failure self‑management and lifestyle modifications
- Conduct staff training and participate in outreach programs to promote heart failure awareness
- Facilitate Advanced Care Planning (ACP) sessions for eligible patients
- Coordinate appointments, vaccinations, screenings, and referrals to other clinics or community services
- Support research, quality improvement initiatives, and programme development
- Mentor, coach, and guide junior staff while fostering a collaborative team environment
Requirements:
- Bachelor’s Degree in Nursing or a relevant health/science discipline
- Registered Nurse with at least 4 years of clinical experience, including 2 years in a relevant specialty
- Strong critical thinking, problem‑solving, and organizational skills
- Excellent communication, interpersonal, and coaching abilities
- Self‑motivated and able to work independently as well as part of a multidisciplinary team
