Band 6 Nurse/Practitioner in Proactive Support Service in Leeds

Leeds Full-Time On-site
J

We are seeking a motivated, compassionate and values-driven practitioner to join Leeds Community Healthcare NHS Trust's new Proactive Support Service.

We welcome applications from Registered Nurses, particularly those with experience in Learning Disabilities, Mental Health, Children's Services or Complex Care, as well as Social Workers and other suitably qualified practitioners with the skills, experience and values to support children and young people with complex needs.

Funded by Leeds Integrated Care Board and delivered in partnership with Leeds City Council's Corporate Parenting Team, this innovative multidisciplinary service supports children and young people at risk of care entry, placement breakdown, or delayed discharge from hospital. The service promotes stability, prevents escalation into care and improves long-term outcomes for vulnerable young people.

The team also supports the Children with No Onward Destination (CNOD) pathway, working with young people who are medically fit for discharge but unable to leave hospital due to placement instability, accommodation issues or complex social circumstances.

This is an exciting opportunity to join a small, developing team where you will help shape a new service, work collaboratively across agencies and make a real difference to vulnerable children and young people.

This role would particularly suit a Registered Mental Health Nurse (RMN) or Registered Learning Disability Nurse (RNLD). However, we also welcome applications from Registered Social Workers and other suitably qualified and registered practitioners who can demonstrate relevant experience and skills in supporting children and young people with complex needs, trauma, behavioural challenges, and placement instability. We are looking for a reflective and innovative practitioner who is passionate about trauma-informed and relational approaches to supporting children and young people. Experience of Dyadic Developmental Practice (DDP), therapeutic parenting approaches, or similar models would be advantageous. Training and/or experience in Positive Behaviour Support (PBS) is also desirable.

The role involves working intensively to support children and young people at risk of placement breakdown, care entry, or delayed discharge through the Children with No Onward Destination (CNOD) pathway. You will provide assessment, consultation, direct intervention and system-wide support, working closely with families, carers, residential providers, social care, education and health colleagues to build resilience, improve stability and reduce crisis. A key aspect of the role is supporting and empowering the networks around the child, helping others understand and respond to behaviours of distress through a trauma-informed and strengths-based approach.

The role is full time but we will consider job shares or part time hours.

Adverts will close early where the maximum number of applicants is reached.
