Our care community teams are based in The Fountains Health Center covering the surrounding geographic area. It's important that you can drive and have access to your own vehicle for work. Mileage expenses are reimbursed in addition to your hourly rate.

As a Community Staff Nurse, the successful candidate will undertake duties involving the delivery of high-quality nursing services and the holistic assessments of patients, ensuring a person-centred approach. You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.

CWP provides health and care services for local people, including mental health, learning disability, community physical health, and all-age disability care - including the provision of three GP surgeries. Our aim is to help people to be the best they can be, adopting a compassionate, person-centred approach to everything we do. We were recently rated as Outstanding for Caring by the Care Quality Commission with a Good rating overall.
