As a vital member of our Clinical Pharmacy Team, you will be responsible for delivering exceptional Clinical Pharmacy Services to patients. The role includes a commitment to dispensaries, ensuring the highest professional standards. In the absence of a more senior Pharmacist, you will take professional responsibility for the dispensary, supervising technicians, assistants, and pre-registration pharmacists. Rotational training across major clinical areas, compliance with Medicines Law, CQC requirements, and internal audit, along with completion of a Clinical Pharmacy Diploma, are integral aspects of this role.
Key Responsibilities
- Prioritize patients based on clinical and pharmaceutical needs.
- Obtain and validate patient medication histories.
- Provide advice to clinicians on necessary changes to patient medication.
- Analyze medicine charts, making appropriate endorsements in line with Pharmacy standards.
- Ensure compliance with VTE and antimicrobial prescribing guidelines.
- Dispense, accuracy check, and hand out prescriptions in line with departmental procedures.
- Communicate medication-related information to patients and healthcare staff.
- Contribute to cost efficiencies by advising on medication usage aligned with formularies or guidance.
- Control pharmacy stock effectively and assist in medication ordering and issuing.
- Participate in clinical trials and conduct Controlled Drugs audits.
- Assist in developments, project work, audit, and surveys as directed.
Communications and Key Working Relationships
- Collaborate with health professionals on the ward to deliver high standards of care.
- Liaise with other pharmacy team members to ensure clinically effective services.
- Refer to more senior pharmacists in the team when appropriate.
- Provide ward cover for other clinical teams or senior colleagues.
Responsibilities
- Handle cash, prescription charges, and issue relevant receipts.
- Sign orders for pharmaceutical products, including controlled drugs.
- Organize work patterns and supervise staff within the framework provided.
- Ensure staff understand and implement relevant procedures.
- Proactively manage risks in accordance with Health & Safety regulations.
- Provide teaching sessions to healthcare professionals and pre-registration pharmacists.
