We are seeking a dedicated Clinical Pharmacist to join our team at NHS Jobs in Swindon. This permanent, full-time position involves patient-facing responsibilities, including structured medication reviews, clinics, and medicines optimization. The successful candidate will collaborate with healthcare professionals, manage long-term conditions, and contribute to projects and quality improvement initiatives.

Responsibilities

  • Conduct structured medication reviews via telephone, video, or face-to-face.
  • Manage long-term conditions, such as blood pressure control.
  • Run clinics, collaborate with healthcare assistants, nurses, and GPs.
  • Educate and support patients in the use of medicines.
  • Handle day-to-day medication issues and support pharmacy technicians.
  • Identify patients suitable for electronic repeat dispensing (eRD).
  • Reconcile medicines on patients from discharge and clinic letters.

Projects and Quality Improvement

  • Collate data for medication optimization in key areas.
  • Conduct audits, searches, and prepare reports with senior clinical pharmacist support.
  • Develop and implement PCN-wide policies and protocols.
  • Facilitate switches of medication in line with local formulary choices.
  • Identify areas requiring new or updated policies and procedures.

Medication Safety

  • Assess and give advice on medication alerts.
  • Produce policies and protocols for practices within scope.
  • Keep a database of document reviews and ensure active review cycles.
  • Identify patients on high-risk medicines and ensure monitoring is carried out.

Training and Development

  • Develop and deliver training for doctors, nurses, and allied health professionals.
  • Complete CPPE PCPEP pathway and become an Independent Prescriber.
  • Provide support and supervision to less experienced pharmacy staff.
  • Facilitate competency assessments of staff within your competence.

Relationships and Networks

  • Develop working relationships with colleagues, community pharmacy, ICB, and hospital and mental health pharmacy colleagues.
  • Participate in meetings to raise the role of the pharmacist and promote medicines optimization.

Strategic

  • Contribute to determining important outcomes and tracking progress.
  • Help ensure evidence is available for regulatory oversight.
  • Promote public health schemes and health equity across the PCN.
  • Lead on aspects of consistent and new ways of working.

Personal

  • Plan own work schedule, engage in clinical and managerial supervision.
  • Adhere to PCN policies and procedures.
  • Personal transport access required for domiciliary visits and other site meetings.

Personal/Professional Development

  • Participate in annual performance reviews and maintain a record of personal/professional development.
  • Take responsibility for own development and demonstrate skills to others.

Quality

  • Alert team members to quality and risk issues.
  • Assess own performance and contribute to the team's effectiveness.
  • Work effectively with individuals in other agencies to meet patient needs.

Communication

  • Communicate effectively within the team and with patients and carers.
  • Recognize alternative communication methods and respond accordingly.

Contribution to the Implementation of Services

  • Apply Practice policies, standards, and guidance.
  • Discuss with the team how policies, standards, and guidelines affect own work.
  • Participate in audits where appropriate.
