We are seeking highly motivated and enthusiastic clinical pharmacists to join our clinical team. If you are an Independent Prescriber (IP) with experience working in General Practice, we would like to hear from you. This onsite role is suitable for candidates seeking permanent employment.
Main Duties of the Job
- Using GP systems (EMIS and S1), you will support practice sites with medicine-related tasks, which may include but are not limited to:
- Discharge support (meds rec)
- General medication reviews
- Supporting patient requests for acute and repeat medications
- Structured Medication Reviews
- Safety audits

Job Responsibilities
You will work as part of a multi-disciplinary team in an onsite role, collaborating with colleagues to resolve medication-related issues, optimize skill mix, improve patient outcomes, and ensure better access to healthcare. You will support practice staff with prescription and medication queries, assist with the repeat prescription system, manage acute prescription requests, and handle medicines reconciliation. Additionally, you will provide expertise in clinical medicines advice while addressing both public and social care needs of patients. You will also support prescribing practices across the PCN, offering clinical leadership on medicines optimization and quality improvement.

- Desirable: Completed CPPE Pathway / currently on CPPE pathway
