Summary:
- Participate and assist in the provision of clinical pharmacy services on a rotational basis.
- Liaise with pharmacy staff, medical, nursing, and other personnel to ensure safe and effective pharmaceutical care.
- Actively prescribe discharge medication according to Trust protocol.
- Undertake clinical and medicine management audits and implement changes for compliance.
- Contribute to quality improvement programs related to pharmacy and medicine usage.
- Provide medicines information service and pharmacy helpline to staff and patients.
- Ensure compliance with pharmacy and medicines laws and regulations.
