Job Purpose
Deliver specialised Clinical Pharmacy Services to the Health and Community Services (HCS), ensuring the safe, clinically effective, and economical use of medicines.
Statutory Responsibilities
The successful candidate must adhere to all relevant States Laws, including the Health and Safety at Work (Jersey) Law 1989, and any associated legislation, Standing Orders, Financial Directions, and other pertinent Codes of Practice. Additionally, adherence to the Data Protection (Jersey) Law is required. Please note that this role has political restrictions.
Job Specific Outcomes
- Assist lead pharmacists in providing a patient-focused medicines optimisation service, adhering to nationally approved standards.
- Monitor and evaluate therapeutic programmes for patients, offering specialist advice on the safe, effective, and economical use of medicines.
- Conduct medication reviews and pharmaceutical care planning, ensuring seamless transitions between care settings and liaising with medical and nursing staff as needed.
- Collaborate within multi-professional health and care teams, offering clinical pharmacy advice and expertise to promote best practices in patient care.
- Provide guidance on complex prescribing issues to ensure compliance with legislative requirements and best practice standards.
- Support the development and implementation of local clinical pharmacy guidelines and policies.
- Deliver clinical and professional supervision within the pharmacy dispensary.
- Develop and conduct medicines-related training programmes for medical, nursing, and pharmacy staff.
Qualifications
Essential:- Vocational (four-year) master’s degree in pharmacy.
- Registration with the General Pharmaceutical Council.
- Post-graduate certificate in Clinical Pharmacy or completion of RPS post-graduate Foundation Programme.
- Post-graduate MSc or Diploma in Clinical Pharmacy.
Knowledge
- Specialised knowledge of therapeutics, pharmaceutical procedures, and medicines governance.
- Understanding of clinical governance, medication incidents, and risk management.
- Up-to-date knowledge of national guidelines and legislation related to pharmacy practice.
- Proficiency in maintaining records according to confidentiality and data protection protocols.
Technical Work-Based Skills
- Proficient in using IT equipment and role-critical software.
- Strong problem-solving abilities.
- Excellent communication skills, both verbal and written.
- Adaptability and ability to influence others effectively.
- Commitment to continuous service improvement and professional development.
Experience
- Minimum of 2 years post-registration experience in hospital practice, including significant clinical pharmacy experience.
- Demonstrated ability to work within multidisciplinary teams and facilitate pharmacy staff development.
