Join the University Hospital of North Midlands NHS Trust as an Advanced Specialist Pharmacist within our Acute Care at Home (ACAH) service. You will play a pivotal role in the development and provision of clinical pharmacy services, specifically within the Community Rapid Intervention Service (CRIS) and Virtual Ward services. This dynamic position will allow you to make a significant impact on patient care, offering both clinical and operational support.
Key Responsibilities
- Clinical Pharmacy Services: Provide expert pharmaceutical care across all aspects of ward-based services, including medicines reconciliation, optimization, and patient monitoring. Depending on patient needs, services may be delivered remotely or through home visits.
- Service Development: Lead the development of efficient medicine supply processes for patients under the care of the Virtual Ward service. Collaborate with multidisciplinary teams to ensure the continual evolution of the pharmacy service in line with emerging needs.
- Expert Advisory Role: Act as an independent prescriber and provide specialized pharmaceutical advice to the CRIS and Virtual Ward teams, ensuring timely and compliant medicine provision.
- Governance and Operational Support: Contribute to the governance and operational functions within the CRIS and Virtual Ward services, working closely with senior leadership to ensure the service meets clinical governance standards.
Qualifications and Experience
- Educational Background: Master’s Degree in Pharmacy or equivalent.
- Experience: Extensive post-registration experience in hospital settings or clinical pharmacy experience in GP practices or intermediate care settings.
- Skills: Strong organizational and leadership abilities, with a commitment to collaborative teamwork within multidisciplinary settings.
