Main Duties of the Job
- Collaborate with a multidisciplinary Antimicrobial Stewardship (AMS) team to drive the AMS agenda forward.
- Support clinicians in the safe prescribing of antimicrobials through ward rounds, addressing ad-hoc queries, and conducting proactive surveillance.
- Enhance your knowledge in antimicrobials and infection management, focusing on pharmacokinetics, pharmacodynamics, spectrum of activity, safety profile, and monitoring requirements.
- Facilitate the transfer of patient care by completing medicines reconciliation in a timely manner and communicating effectively with GPs and community pharmacy teams.
- Accurately convey medicine-related information to healthcare professionals and patients, including those with communication challenges, to promote shared decision-making.
- Assist OUH dispensaries and liaise with external agencies, such as aseptic production services or homecare providers, to ensure the safe and timely supply of medications.
