Aughton Surgery seeks an experienced prescribing Clinical Pharmacist for a part-time position of 20 hours per week. This role offers flexibility with the possibility of remote work, although some in-person presence at the surgery is required. Full in-person working hours can also be accommodated. The successful candidate will work within their professional boundaries, collaborating with a team of pharmacists and other healthcare professionals to enhance patient health outcomes and primary care efficiency. The role is integral to the general practice team, contributing to medication optimization and supporting clinical staff in patient management. An independent prescribing qualification is essential for this position.

Main Duties

  • Prescription Management: Address queries raised by the prescriptions coordinator and manage repeat prescriptions.
  • Medication Reviews: Ensure patient medication reviews are current.
  • Medicines Reconciliation: Process clinical correspondence related to medicines.
  • Structured Medication Reviews: Conduct detailed reviews of patient medications.
  • Task Management: Respond to and action tasks from clinical and administrative staff.
  • MHRA Alerts: Act on MHRA drug alerts.
  • New Patient Medication Review: Review and re-authorize medications for newly registered patients transferred via GP2GP.
  • Blood Pressure Management: Oversee patient blood pressure control.
  • Chronic Condition Management: Handle medication management for patients with long-term conditions.
  • Dose Adjustments: Amend medication doses based on blood test results, such as Levothyroxine adjustments.

Note: This list is not exhaustive, and additional duties may be assigned as needed by the surgery.

Aughton Surgery is a collaborative and supportive environment with a diverse team. Our practice comprises five partners (three GPs, one Practice Manager, and one Finance Manager), three Salaried GPs, one Physician Associate, one Practice Nurse, and one HCA. As a training practice, we regularly host GP registrars, medical students, and student PAs. Our clinical team is supported by a knowledgeable and friendly admin/reception team, making Aughton Surgery a pleasant place to work.
