Join our team to deliver a high-quality Medicines Optimisation Service, enhancing medicines-related outcomes for high-risk patients within the frailty Hospital @ Home caseload. Collaborate with fellow Hospital @ Home pharmacists to ensure standards transcend services. Build relationships with key stakeholders, engaging with medical teams, clinics, PCNs, and GPs. Conduct clinical medication reviews in patients’ homes, ensuring proactive liaison with healthcare professionals regarding medication changes.
Responsibilities
- Make prescribing recommendations, case manage patients with frequent acute admissions due to medicines-related issues, and support the identification of high-risk patients post-hospital discharge.
- Provide professional support, compose patient-agreed medication care plans, and collaborate with health and social care professionals.
- Work with the frailty Hospital @ Home MDT for optimal patient care.
- Devise practical solutions for patients to self-administer medication, enhance adherence, and liaise with North Central London ICB Pharmacists for consistency in local prescribing strategies.
- Collaborate with system partners to improve medicine pathways, ensuring alignment with organizational priorities.
- Contribute to meetings, represent pharmacy, and respond promptly to urgent requests.
- Deliver training sessions, identify areas of risk in case management, and ensure compliance with medicines legislation.
- Collect data for service evaluation, providing information on parenteral drug administration, and participate in antibiotic stewardship.
- Respond to medicines information queries and support the Lead Pharmacist for the Hospital @ Home service.
