The Clinical Pharmacist will play a crucial role within the Vine clinical team, working to enhance the safety, quality, and outcomes of prescribed medicines. As a key member of the practice team, you will be a point of contact for patients regarding medication queries and act as a liaison between various pharmacists, supporting medication reviews, and ensuring continuity of care for patients.
Key Responsibilities:
- Conduct patient-facing clinics and medication reviews, optimizing medicine use for long-term conditions.
- Manage care home and domiciliary clinical medication reviews, improving safety and administration procedures.
- Provide medicines support through patient-facing clinics and a telephone help line.
- Respond to medicine-related inquiries from healthcare professionals and patients, offering recommendations.
- Contribute to reducing unplanned hospital admissions through audit and individual patient reviews.
- Reconcile medicines post-hospital discharge, ensuring continuity of supply for high-risk patient groups.
- Collaborate with community and hospital pharmacy colleagues, facilitating referral processes.
- Manage repeat prescribing reauthorization process, including review of patient requests and medication dates.
- Identify high-risk patient cohorts through computer searches and contribute to service development.
- Analyze and present medicines data to support decision-making and undertake clinical audits.
- Implement changes to medicines based on MHRA alerts, withdrawals, and local/national guidance.
- Provide education and training on therapeutics and medicines optimization for the healthcare team.
- Ensure practice compliance with CQC standards related to medicines.
- Support public health campaigns and provide specialist knowledge on public health programs.
