The NHS is seeking a skilled Pharmacy Technician to join our team in Eastleigh. This role involves providing specialist pharmaceutical services and professional advice to a diverse group of healthcare professionals, including Practice GPs, Nurses, Pharmacists, and HCAs. You will play a crucial part in ensuring the safe, effective, and cost-efficient use of medicines.
Key Responsibilities- Medicines Reconciliation: Perform medicines reconciliation following local protocols, ensuring accuracy in patients’ medical records and prescriptions. Any discrepancies should be referred to the clinical pharmacist or GP.
- Patient and Staff Liaison: Communicate effectively with patients, clinical and non-clinical staff, practice and community pharmacists, technicians, and carers to enhance pharmaceutical care.
- Support Clinical Pharmacists: Assist clinical pharmacists in implementing systems for repeat prescriptions, medicines optimisation, and reducing medicines waste. Ensure the maximisation of patient outcomes through structured medication reviews, particularly for those on multiple medications or in care homes.
- PCN Directed Enhanced Service (DES): Aid in the practice’s implementation of the PCN DES, supporting the delivery of comprehensive and integrated pharmaceutical care.
