We are seeking a dedicated Clinical Pharmacist to join our dynamic team at NHS Jobs. This is a developmental role tailored to your experience, knowledge, skills, and competencies. The successful candidate will provide essential support to senior clinical pharmacists across a variety of services.

Key Responsibilities

  • Support Services: Work with Chemotherapy/Haematology Services, Mental Health Services, Urgent Care Services, Medicines Advice services, Patient Services, and various wards on a rotational basis.
  • Medicines Reconciliation: Conduct medicines reconciliation and drug history transcribing in accordance with Trust standards.
  • Discharge Medicines Facilitation: Ensure the efficient management of discharge medicines, meeting the needs of patients and general practitioners.
  • Liaison: Maintain clear and timely communication with pharmacy staff, nursing staff, Allied Health Professionals, medical staff, and GPs.
  • Clinical Management: Provide support and supervision to pre-registration pharmacists, clinical placements, and undergraduate vacation pharmacy students.
  • Professional Development: Engage in professional development opportunities, potentially including palliative care and paediatrics.
  • Performance Appraisal: The Lead Clinical Pharmacist will conduct an annual performance appraisal, reviewing job description, development and training needs, performance over the past year, and objectives for the following year.
