Clinical Practice
- Conduct prescription reviews and provide medicines-related advice to healthcare professionals and patients.
- Perform medication reconciliation at admission and/or discharge, tailored to each rotation’s processes.
- Offer medicines-related counseling to patients or their caregivers to enhance medication understanding and adherence.
- Monitor and influence the prescribing practices of healthcare professionals to ensure safe and effective medication use.
Training and Supervision
- Actively engage in the Foundation Programme to attain defined skills and competencies.
- Pursue continuing professional development, including postgraduate clinical diploma level study.
- Supervise dispensary operations to ensure safe and effective prescription dispensing, clinical screening, and final checking.
- Provide training and supervision for pre-registration pharmacy graduates and pharmacy students, serving as a role model in the field.
Management of Resources
- Collaborate with pharmacy technicians and assistants to support discharge facilitation and the reuse of patients’ own medicines.
- Adhere to local formulary and medicines-related guidance for cost-effective medication usage.
- Organize and prioritize workload in alignment with rotation specialty requirements.
Quality Improvement and Audit
- Lead quality improvement projects and audits focused on medicines optimization and prescribing.
