Duties include administering medicines, conducting medication reviews, and reconciling medications. Utilize consultation skills to work with patients for optimal medication use. Support medication reviews for new care home patients, synchronize medicines for patient transfers, and liaise with community pharmacies. Provide expertise in public health, social care, and address local health inequalities. Collaborate with Clinical Pharmacists for complex cases and contribute to achieving QOF, IIF, and other locally commissioned enhanced services.
Duties and Responsibilities
- Undertake patient-facing and patient-supporting roles
- Ensure effective medicines use through shared decision-making conversations
- Administer medicines, including checking inhaler technique
- Support medication reviews and medicines reconciliation
- Utilize consultation skills for partnership with patients
- Support Clinical Pharmacist in Structure Medication Reviews (SMR)
- Provide expertise for public health and social care needs
- Manage shared care protocols and liaise with Clinical Pharmacists
- Support the PCN to deliver on QOF, IIF, and other enhanced services.
Location: Wirral, CH49 0AB. Salary: £28,000.00 to £30,000.00 per year. Hours: Full time. Closing Date: 31 January 2024. Job Type: Permanent.
