Key Responsibilities:

  • Providing clinical services to wards, the Hospice, and associated departments following professional, departmental, and hospital policies.
  • Promoting responsible, safe, and cost-effective prescribing to optimize clinical outcomes and minimize adverse reactions and errors.
  • Offering support to medical, nursing, and pharmacy staff, addressing queries related to individual patients and hospital protocols.
  • Conducting clinical audit projects involving the use of medicines within the hospital.
  • Identifying changes in prescribing practices affecting the drugs budget and advising the Chief Pharmacist.
  • Providing professional, managerial, and clinical leadership to all pharmacy staff.
  • Ensuring compliance with legal, ethical, and other applicable standards in designated areas of responsibility.
  • Recording clinical interventions and activities regularly in line with departmental policy.
  • Attending Multidisciplinary Team Meetings (MDTs) and relevant group meetings as required.
  • Participating in Saturday, late, and on-call rotas as necessary.
  • Contributing to the departmental staff appraisal program and monitoring professional development.

Preferred Skills and Experience:

  • Minimum of 2 years in a clinical pharmacy role within a hospital setting.
  • Experience in palliative care.
  • Post-registration experience within a hospital setting.
  • Substantial experience in multidisciplinary communication.
  • Development of policies, procedures, and guidelines, as well as report writing and presentation skills.
  • Accurate clinical, dispensing, and checking abilities.
  • Aware of drug cost issues.
  • Medication counseling experience.
  • Multidisciplinary collaboration with nursing and/or medical colleagues.
  • Experience with private patients.
  • Proficiency in analyzing and applying complex clinical data and financial software.
