NHS Jobs is seeking a motivated and passionate Clinical Pharmacist to join our team in Maidenhead. As a key member of the general practice unit, the successful candidate will operate within their professional boundaries, collaborating with a team of pharmacists. Supported by a Senior Clinician and Line Manager, the post holder will play a crucial role in chronic disease management within PCN practices.
Responsibilities:
- Conduct structured medication reviews to proactively manage patients with complex polypharmacy.
- Provide primary support to general practice staff, addressing prescription and medication queries.
- Assist in managing the repeat prescription system, handle acute prescription requests, and conduct medicines reconciliation during transfers of care.
- Offer expertise in clinical medicines advice, addressing both public and social care needs of patients in GP practice(s).
- Provide clinical leadership on medicines optimization and quality improvement.
- Manage aspects of the quality and outcomes framework and enhanced services.
- Ensure integration with community and hospital pharmacy for improved skill mix, patient outcomes, and healthcare access.
- Support the development of non-medical prescribing skills.
Requirements:
- Registered Pharmacist with relevant professional qualifications.
- Experience in chronic disease management and structured medication reviews.
- Strong interpersonal skills and ability to work effectively in a multi-disciplinary team.
- Passion for delivering excellent service within general practice.
- Motivated to contribute to the improvement of quality of care and operational efficiencies.
