Key Responsibilities
- Conduct clinical structured medication reviews.
- Manage patients with common/minor/self-limiting ailments within your scope of practice.
- Offer advice to the Prescription Admin Team and address patient inquiries.
- Reconcile medicines following discharge, identifying and rectifying changes.
- Set up systems for continuity of medicines supply to high-risk patients.
- Refer patients to appropriate healthcare professionals for timely care.
- Support repeat prescribing reauthorization and identify high-risk patient cohorts.
- Contribute pharmaceutical advice for the development of new services.
- Analyze and present medicines data to support decision-making.
- Undertake clinical audits of prescribing and implement changes.
- Monitor practice prescribing against local health policies and guidance.
- Provide newsletters and bulletins on important prescribing messages.
- Deliver education and training on therapeutics and medicines optimization.
- Ensure compliance with CQC standards where medicines are involved.
- Support public health campaigns and provide specialist knowledge.
