The Pharmacy Technician will play a critical role within the LSMP & TLS PCN Pharmacy Team, operating under the guidance of the Senior Clinical Pharmacist. This position ensures the safe, accurate, and timely provision of prescribed medications to patients.
Key responsibilities include:
- Medication Supply Management: Ensuring the precise and prompt supply of prescribed medications to patients.
- Technical and Administrative Support: Assisting clinical pharmacists and clinicians with their daily tasks.
- Structured Medication Review: Contributing to the implementation of the DES specification for medication reviews and optimization.
- Medication Queries and Reconciliation: Addressing medication-related queries, managing acute prescription requests, and performing medicines reconciliation post-discharge from other care providers.
- Patient Safety Audits: Conducting audits to ensure patient safety in medication prescribing, and presenting findings to the clinical team.
- Liaison and Coordination: Working closely with patients, clinical and non-clinical staff, practice and community pharmacists, hospital staff, and patient carers to enhance pharmaceutical care.
