Main Duties of the Job

As a Highly Specialist Pharmacist, you will be responsible for providing a highly specialized clinical pharmacy service to the agreed specialty, including:

  • Taking drug histories, completing medicines reconciliation, and screening patients for medication-related issues promptly after admission.
  • Following up with patients post-discharge, ensuring communication of changes in medications to the GP, and patient awareness of any changes.
  • Assessing patient needs for medication, determining optimal choices, and effectively communicating potential interactions, contraindications, or cautions to clinicians.
  • Providing highly specialized pharmaceutical advice to clinicians, healthcare professionals, and patients, both within the Trust and externally as needed.
  • Demonstrating advanced levels of clinical reasoning and judgment, utilizing persuasive skills when communicating with medical staff, and ensuring consultants are informed about evidence underpinning recommended treatment choices.
