Summary:

As a Clinical Pharmacist, you will play a crucial role in clinically assessing and treating patients with expertise in specific disease areas. Your responsibilities will include providing specialist knowledge on medicines, offering medication reviews, and delivering health education advice to patients through face-to-face, telephone, or notes-based reviews, as agreed with the practice.

Key Responsibilities:

  • Medication Reviews: Conduct comprehensive medication reviews, ensuring the safe and effective use of medicines by patients.
  • Health Education: Provide valuable health education advice to patients, addressing concerns and queries related to medicines, side-effects, and dosage.
  • Leadership: Demonstrate leadership in person-centred medicines optimisation and contribute to quality improvement initiatives.
  • Communication: Process specialist treatment letters and hospital medication discharge letters efficiently, maintaining accurate records.
  • Patient Support: Act as a key contact in the practices, addressing and resolving all medication-related queries from patients.
  • Clinical Care Improvement: Support improvements in clinical care by actively participating in practice-based audits and implementing necessary changes.

Qualifications and Requirements:

  • Registered Pharmacist with the General Pharmaceutical Council (GPhC).
  • Clinical experience in assessing and treating patients with a focus on specific disease areas.
  • Strong knowledge of medicines, including side-effects, dosage, and medication information.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Commitment to ongoing professional development.
