Join Bury GP Federation, an organization committed to ensuring health equality for all. We provide clinical pharmacy services to three Primary Care Networks (PCNs) in Bury and are part of the Local Care Organisation, working closely with our partners in health and social care to deliver integrated services for our patient population. We are currently seeking a Senior Pharmacist to enhance our PCN Pharmacy team. The ideal candidate will have completed the CPPE Pathway (or equivalent) and be a Qualified Prescriber.

This role involves providing practice-level support, delivering enhanced services to care home residents and housebound patients, and identifying other patient cohorts for structured medication review. The Senior Pharmacist will also mentor Pharmacists new to Primary Care and support the growth of our Pharmacy Technician service, in collaboration with the University of Manchester to support Trainee Pharmacists and Pharmacy students.

Main Duties and Responsibilities

  • Leadership and Mentorship: Provide leadership, mentorship, and supervision for the Pharmacy team, including Pharmacists and Pharmacy Technicians, across practices within the network.
  • Clinical Competence: Maintain clinical competence as a Prescriber, manage own caseload, conduct clinical medication reviews, and proactively manage patients with complex polypharmacy, particularly frail elderly and vulnerable adults.
  • Medicine Management: Support medicine reconciliation post-discharge, structured medication reviews, cardiovascular disease interventions, and clinical governance work. Ensure medicine optimisation, safety, antibiotic stewardship, and de-prescribing agendas are integrated into medication reviews.
  • Collaboration: Work within a multidisciplinary team, integrating with wider healthcare teams, including Community and Hospital Pharmacy, and Intermediate Care colleagues. Communicate and implement medication changes as necessary.
