An exciting opportunity awaits as the Intermediate Care Pharmacy team expands with comprehensive funding, reaching all boroughs within the Gwent Region. This initiative includes the integration and redesign of Pharmacy services for Community hospital rehabilitation wards in South Monmouthshire and Torfaen. As the role of the Clinical Pharmacy Technician evolves, we strive to enhance skill-mix, enabling Pharmacists to operate at the forefront of their expertise.

Key Responsibilities

  • Collaborate within a diverse multidisciplinary team, promoting patient-centered care.
  • Display resilience, adaptability, and a genuine passion for contributing to service development.
  • Practice within a designated borough (Caerphilly, South Monmouthshire, or Blaenau Gwent), offering remote and in-person cover to other boroughs when required.
  • Conduct home visits, emphasizing personalized patient care.
  • Be prepared to travel, with access to personal transport (travel expenses reimbursed).
