We're seeking a compassionate Senior Health Care Support Worker to deliver person-centred care at clients' homes. You'll support independence, dignity, and wellbeing, provide personal care, medication assistance, meal prep, mobility support, household tasks, companionship, and post-hospital care. You'll also supervise carers, monitor health, maintain confidentiality, and follow safety standards while contributing to a high-quality, supportive service.
Responsibilities
- Provide personal care and support to clients in their homes
- Administer or assist with medication as required and in line with care plans
- Prepare meals and support healthy eating as part of daily routines
- Assist with mobility and daily living tasks to promote independence
- Carry out household tasks to support a safe and clean living environment
- Provide companionship and social support to reduce isolation
- Support post-hospital care and coordinate with other health professionals as needed
- Supervise carers and ensure adherence to care plans and standards
- Monitor health and wellbeing, report changes to the appropriate team
- Maintain client confidentiality and observe safety standards
- Contribute to the delivery of a high-quality, supportive service
