At Christies Care, we are recruiting a Care Coordinator to support the delivery of outstanding hourly care services in Jersey.
Responsibilities
- Coordinate and manage rotas to ensure continuity of care
- Build strong relationships with clients, families and healthcare assistants
- Set up, review and monitor care packages, liaising with funders including LTC
- Respond to last minute changes and provide on call support on a rota basis
- Deliver care when required
- Maintain accurate records in line with JCC standards
Requirements
- Minimum 12 months care experience
- Level 3 qualification in care essential
- 20 days annual leave plus bank holiday
