We are neighbors helping neighbors.We are looking for a talented and compassionate Physical Therapist, Per Diem to work for our NVNA Works private duty sector. In this position, you will provide physical therapy services to clients as requested and reflective of the philosophy, mission and standards established by the Agency.What will you be doing:Provides professional physical therapy services/treatments as requested.Evaluates the client’s functional abilities and physical potentials as well as performs diagnostic and prognostic evaluations (muscle, nerve, joint) to assist the client in evaluating the level of function. Assessment data documented in the clinical record is timely, accurate, legible and complete.Assists client to develop/restore function, maintain maximum potential and achieve adequate pain control.Documents and reports client outcomes and responses to treatments and/or any other changes in the client’s condition in a timely manner.Develops, implements and records teaching plan in the care of the client’s total physical therapy program; instructs client, family members and/or significant others and health care team personnel in the patient/client’s total physical therapy program including recommendations for physical/environmental safety and in the use of prosthetic, orthotic and assistive devices.Participates in the development of the clients’s plan of care, problem list and summaries and documentation requirements.Documents, according to Agency policies and procedures, the type of treatments and client outcomes in the narrative notes.Provides supervision of the plan of care for Physical Therapy Assistant.Orders/procures patient equipment through approved medical supply vendors.Identifies client/family needs for various services and makes appropriate community and Agency referrals.Participates in community education programs as appropriate; participates and/or initiates case conferences as the client situation demands.
