Community experience is essential.The team has a breadth of experience and a shared goal to offer the highest standard of care to our patients. We work closely with other health and social care professionals across primary and secondary care, as well as with patients, families and carers in their own home.Your role will be to lead the team and case manage the provision of care within an individual\’s home or residential home, including palliative care and long term conditions management. You should have experience in caseload management and be able to prioritise work. The role is diverse, challenging and extremely rewarding.
