The co-ordinator will workpredominantly in the Urgent and Emergency Care arenas within the Hospital,alongside the multi-disciplinary teams within those environments. TheCo-ordinator will be part of the Hospital Discharge Team which support dischargeand patient flow, focusing on the needs of the frail person. Main duties of the jobFrailty Information Care Co-ordinator Based at: Diana Princess of Waleshospital, Grimsby. Hours: 37.5 hrs Fixed term for 12 months PredominantlyMonday to Friday but able to flexible to meet the demands of the role Range 4 £25,498 – £27,202 Per Annum. You will appointed on a fixed salary point within the range. This is a new and exciting role basedat Diana Princess of Wales hospital. The co-ordinator will workbetween multiple organisations involved in the health, social and holistic careof patients who are recognised as frail, extracting and coordinatinginformation for frail patients on attendance, to help inform clinical decisionmaking and care planning for frail individuals, including redirection back tothe community. The information gained from the patient, family, carers,electronic patient information records will help to identify and inform theright care by the right person at the right time to ensure best outcomes forthe individual. The Co-ordinator will be a strong teammember, demonstrating ability to work with a wide variety of people atdifferent levels of seniority and from different professional disciplines aswell as working independently. Attentionto detail, digital literacy and communication skills are vital in this role. This is an opportunity to be involved in influencing anddeveloping healthcare frailty pathways. Person SpecificationQualificationsFoundation Degree or Higher Education Certificate (level 4) in Integrated Care or other appropriate topic OR Level 3 in health or social care, plus 2 years experience in this field and with willingness to work towards level 4 on appointment Literacy and numeracy level 2 qualification OtherAbility to rearrange and prioritise workload to meet the needs of the service Willing to undertake appropriate professional development Ability to demonstrate commitment to CPG core values Flexible working that may include occasional weekends to meet the needs of the service Skills and KnowledgeAbility to effectively manage own workload Effective interpersonal and communication skills, both face to face and over the phone Ability to liaise effectively with people at all levels Effective customer care skills Ability to maintain confidentiality Ability to demonstrate empathy and sensitivity when dealing with clients/patients.
