Tripod Partners are looking for 2 Independent living Assessors (ILA)/ Occupational Therapy Assistant\’s to work for a council in Central London. ResponsibilitiesCarry out functional assessments to evaluate service users’ ability to manage daily living tasks safely and independently.Coordinate and monitor cases which may include reablement programmes, setting goals and reviewing progress weekly to reduce dependency on services.Hold a caseload with weekly allocations, ensuring timely and effective support planning and delivery.Arrange and fit equipment which may include specialist equipment and identifying minor and simple major adaptations, to promote independence at home.Support safe manual handling and transfers, under guidance from qualified OT and physiotherapy staff.Develop outcome-focused support plans for individuals.Process referrals and complete discharge assessments, identifying ongoing needs and eligibility under the Care Act 2014.Liaise with multidisciplinary teams and external agencies, ensuring coordinated care and timely service delivery.Promote the use of assistive technology, identifying opportunities to enhance independence and reduce reliance on statutory services.Maintain accurate records and contribute to performance monitoring, safeguarding compliance and service quality.Must have experience working as a Occupational Therapy AssistantMust have UK experiencePreferably experience working for a councilPlease apply to hear more details.
