Are you an Administrator looking for a community job in a developing service? Our Community Transfer of Care Hub (CToCH) service is looking for an enthusiastic administrator to support the clinical and admin teams. There is a focus on data collection to evidence the difference new ways of working are having on flow through the system, bed days saved and improved patient experience.
The post will require you to work in close collaboration with colleagues from acute hospitals, social care, primary care, Pathway 2 home providers and our Integrated Care Board (ICB) and really make a difference for these service users and their families. We are developing new ways of working alongside acute hospital and local authority colleagues to support the service users admitted to acute hospital beds to leave hospital in a safe and timely way and maximise capacity of community resources. The Service currently operates 08:00 - 18:00, 7 days a week including Bank Holidays.
Main duties of the job
This role supports the delivery of the admin component within the Community Transfer of Care Hub (CToCH). As a CToCH administrator, you will work alongside the Admin lead to provide comprehensive admin support to health and social care professionals, hospitals, GPs, service-users, their relatives and carers and other stakeholders both within Sirona and in the wider health community. You will be required to answer telephone enquiries in a friendly and professional manner, ensuring effective and accessible communication with staff, service users, the general public and any others as appropriate. This admin role involves maintaining accurate and up to date patient records and data collated in the Sirona IT systems the electronic patient record, databases and locally held spreadsheets.

Person Specification
- Functional literacy and numeracy level 3/ GSCE grade A-C or equivalent including English Language and Mathematics
- NVQ 3 Business and Administration or equivalent experience
Skills, knowledge and experience
- Ability to take responsibility for tasks and to work autonomously if needed
- Ability to manage time, work under pressure and build efficient working practices
- Experience of working in a demanding office environment and meeting deadlines
- Trained in a range of systems /IT Clinical System and Microsoft Office
- Able to handle sensitive and confidential issues and recognise the need for discretion and confidentiality
- Previous acute or community care experience
- Proven ability to maintain effective workload prioritisation
- Previous experience of Cloud based telephony systems
- Previous use of EMIS
