About the Role
You will support young people aged 18-25 who are homeless or at risk of homelessness to access services, build skills, and improve their social, emotional, and practical wellbeing. Working both within our services and the wider community, you will deliver engaging activities, promote independence, and support positive move on outcomes. You will play a key role within the Team Around the Young Person framework, working collaboratively with partners to deliver high quality, person centred support.
What you'll be doing
- Plan and deliver meaningful activities that improve wellbeing and promote independence
- Manage a caseload, providing person centred support tailored to individual goals
- Work collaboratively with internal teams and external partners
- Engage community services to enhance opportunities for young people
- Promote safeguarding and adhere to policies and procedures
- Record outcomes, monitor progress, and contribute to KPI reporting
- Support young people to develop skills and achieve sustainable move on
About you
You'll be confident, organised, and passionate about supporting young people to succeed, with the ability to deliver engaging activities and meaningful support. We are looking for someone who can demonstrate:

- Strong communication and interpersonal skills

- A trauma informed and solution focused approach
