An exciting opportunity has arisen for a Senior Case Manager at the Humber and North Yorkshire Health and Care Partnership Mental Health, Learning Disabilities and Autism Collaborative. Please ensure approval for secondment is sought prior to application being submitted. This is a part time post of 22.5 hours (3 days per week).
Main duties of the job:
- Work as part of a dynamic team delivering an effective service.
- Support managers and staff across Humber and North Yorkshire to ensure safe and effective care pathways are in place for service users of out of area hospital placements within the NHS and independent sectors, including rehabilitation, adult and older adult acute and PICU placements.
- Undertake face to face quality visits, which may include travel outside of the Humber and North Yorkshire region.
- Agree on robust discharge plans in line with national guidance.
- Liaise with key partners from the system to ensure local services can meet individual need and oversee the repatriation of individuals.
Skills and Competencies:
- Demonstrate an understanding of the requirements of quality in healthcare and commissioning healthcare services.
- Ability to negotiate on difficult and controversial issues including performance and change.
Qualifications, Education & Training:
- Registered Mental Health / Learning Disability Nurse or other registered health or social care professional.
- Educated to master's level or equivalent level of experience of working at a senior level in specialist area.
- Prince 2 and Managing Successful Projects.
Knowledge & Experience:
- Experience of managing clinical risks and reporting.
- Experience of drafting briefing papers and correspondence for a senior audience.
- Experience of working in quality assurance / quality improvement.
