We are seeking a passionate IP Clinical Pharmacist with experience of working in general practice to join our dynamic team. You will be an independent prescriber within your clinical competencies, providing expertise in clinical medicines management across medication‑related queries, acute prescription requests and hospital discharge prescribing arrangements. You will also streamline processes, develop protocols for safer prescribing, and meet CQC expectations.

Key Responsibilities
  • Act within professional boundaries, supporting and working alongside a team of pharmacists and pharmacy technicians in general practice.
  • Work as part of a multi‑disciplinary team in a patient‑facing role.
  • Takes responsibility for areas of chronic disease management within the practice and undertakes clinical medication reviews to proactively manage patients with complex polypharmacy.
  • Provides primary support to general practice staff with regard to prescription and medication queries, including repeat prescriptions, acute prescription requests, medicines reconciliation on transfer of care, and safer prescribing systems.
  • Provides clinical leadership on medicines optimisation and quality improvement, managing aspects of the Quality and Outcomes Framework and enhanced services.
  • Ensures integration with community and hospital pharmacy to leverage skill mix, improve patient outcomes, ensure better access to healthcare, and help manage workload.
  • Act as a specialist resource to answer medicines‑related queries, signpost to other services, and liaise with other healthcare professionals as required.
  • Streamline processes and develop protocols for safer prescribing to meet CQC expectations.
  • Use initiative to gather information and utilise resources such as prescribing guidelines.
  • Demonstrate an understanding of, and contribution to, the workplace vision.
  • Demonstrate ability to improve quality within the limitations of the service.
Qualifications
  • Experience working in general practice as a Clinical Pharmacist.
  • Independent prescriber within clinical competencies.
  • Proficiency in SystmOne electronic prescribing system.
  • Excellent communication skills and proactive engagement with patients, carers, colleagues and other agencies.
  • Ability to work independently, adapt to change, and use prescribing guidelines and other resources.
  • Good level of competence in SystmOne required.
