The admission avoidance team is recruiting a Band 7 Advanced Nurse Practitioner with a passion for caring for people in their own homes and promoting independence through collaborative working. You will identify and manage deterioration, work across inpatient and community teams, and have strong organizational skills. The Intermediate Care team supports training and education; the successful candidate will be expected to provide teaching regularly and may receive in-house training and CPD opportunities as appropriate.

Main duties of the job

  • The Advanced Nurse Practitioner will hold a caseload, manage follow-up, and undertake initial assessments on patients in the community.
  • They will work closely with the MDT to ensure safe and effective practice and that patients are seen within the required time frame.
  • Demonstrate critical thinking and act within professional boundaries to provide care from initial history taking through clinical assessment, diagnosis, treatment and evaluation.
  • Maintain patient safety through sound clinical skills and effective risk assessments.
  • Work autonomously in decision-making based on assessment, diagnosis and interpretation of test results.
  • Ensure the best possible clinical outcomes by using up-to-date skills and adhering to evidence-based practice and relevant policies and procedures.
  • Ensure patients have a positive experience by respecting, empowering and partnering with them throughout the care planning process.

  • Qualifications and Training
    • Registered General Nurse, Adult Branch and current NMC Registration
    • Advanced physical assessment with history taking (level 7)
    • Commitment to complete Independent Nurse Prescriber course
    • Experience and evidence of teaching and assessing (e.g., Preceptorship/Mentor)
    • Full UK driving licence and access to a car
    • Accredited Advanced Practitioner
    • Non-Medical Prescribing (level 7)
  • Experience
    • Experience of reviewing deteriorating patients
    • Experience of providing patient-facing interventions
    • Ability to interpret biochemical results and lead on recruitment
    • Experience of working with Nursing, Physiotherapy and Occupational Therapy teams in inpatient and/or community settings
    • Experience of providing clinical information to the wider MDT
