The Advanced Nurse Practitioner will provide advanced clinical care including assessment, diagnosis, treatment and management of patients presenting with undifferentiated, acute and long-term conditions. You will undertake independent prescribing, request and interpret investigations, and make appropriate referrals to other services.

We are a well-established GP practice working within a collaborative Primary Care Network, serving a diverse and supportive local community. Our team includes GPs, ANPs, practice nurses, pharmacists, care navigators and an experienced administrative team who work closely together in a positive and respectful environment. The Advanced Nurse Practitioner (ANP) will work as an autonomous practitioner within the primary care team, delivering high-quality, evidence-based care to patients. The post holder will assess, diagnose and manage patients with a range of acute, chronic and complex conditions, contributing to the delivery of safe, effective and patient-centred services.

Clinical Responsibilities
  • Undertake comprehensive clinical assessment, diagnosis and management of patients presenting with undifferentiated, acute and long-term conditions.
  • Provide independent prescribing in line with scope of practice and current legislation.
  • Request, interpret and act upon appropriate investigations.
  • Make referrals to secondary care, community services and other professionals as required.
  • Manage same-day demand, urgent appointments and planned care clinics.
  • Provide health promotion, disease prevention and patient education.
  • Maintain accurate, timely and comprehensive clinical records.
Professional Responsibilities
  • Work autonomously within agreed clinical guidelines and protocols.
  • Practice in accordance with the NMC Code of Professional Conduct.
  • Maintain professional registration, revalidation and CPD requirements.
  • Participate in clinical supervision, appraisal and peer review.
Quality, Governance and Service Development
  • Contribute to clinical governance, audit and quality improvement activities.
  • Support the development and review of policies, protocols and pathways.
  • Contribute to learning, teaching or mentoring of staff where appropriate.
  • Promote a culture of safety, learning and continuous improvement.
Team Working
  • Work collaboratively with GPs, nurses, pharmacists, allied health professionals and administrative teams.
  • Support integrated working across the Primary Care Network.
  • Contribute positively to team effectiveness, communication and morale.
Other Duties
  • Undertake any other reasonable duties consistent with the role and level of responsibility.

Person SpecificationExperience
  • Experience working at an advanced practice level in primary care, urgent care or a similar clinical setting.
  • Experience of autonomous clinical assessment, diagnosis and management of patients.
  • Experience of managing patients with acute, chronic and complex conditions.
  • Experience of using clinical systems and maintaining accurate patient records.
  • Experience of working within general practice or a Primary Care Network.
  • Experience of supervising, mentoring or supporting other clinicians.
  • Involvement in audit, quality improvement or service development.
Qualifications
  • Registered Nurse with current NMC registration.
  • Masters degree (or working towards) in Advanced Clinical Practice or equivalent.
  • Evidence of continuing professional development relevant to advanced practice.
  • Full MSc in Advanced Clinical Practice.
  • Additional relevant clinical qualifications (e.g. minor illness, chronic disease management).

  • Ability to work independently within scope of practice and recognise own limitations.
  • Strong communication and interpersonal skills.
