We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our practice team. The ideal candidate will play a crucial role in optimising patient care through effective medication management and collaboration with healthcare professionals. This position requires a strong understanding of pharmacology and patient care principles, particularly in a General Practice setting.

Main duties of the job:
- To play a pivotal role in the delivery of high-quality primary health care.
- To independently assess, verify and prescribe for patients safely within national and local guidelines.
- To provide clinical medication reviews for patients with single or multiple long term conditions where medicines optimisation is required.
- To reconcile medicines following discharge from hospitals or intermediate care and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge and to reduce risk of readmission.
- To maintain full and complete records of all patient contacts using appropriate clinical templates and coding.
- To undertake a proactive role in audit and quality improvement implementing recommendations where appropriate.
- Provide independent information to patients with regard to medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate.
- Work with practice team to ensure full compliance with Care Quality Commission standards for safe and effective care.

We are looking to expand our Clinical Pharmacy team. Our Lead Clinical Pharmacist is a Partner in the practice, and we employ a further clinical pharmacist, pharmacy technician and have our own in house medicines management support. In addition, we have PCN Clinical Pharmacist support 3 days per week. Our colleagues value our contribution, and we are continually expanding our offering. We are committed to training and developing our workforce and support is available. Serving a patient population of c15,000 in a modern purpose-built premises in the heart of Ilkeston, Derbyshire, the surgery also has two branch surgeries in the neighbouring areas of Cotmanhay and Kirk Hallam.

- Hours: Ideally Total 26.5 hours a week, to be worked as follows: Monday 8:00-18.00 (with 30 minutes unpaid break) - COT, Tuesday 9:00-18:00 (with 30 minutes unpaid break)- OSS, Friday 9:00-18:00 (with 30 minutes unpaid break)- OSS. However, we will be prepared to offer flexibility for the right candidate.
Qualifications:
- Registered Pharmacist with the GPhC.
- Qualified as an Independent Prescriber.
- Minimum of 2 years post-qualification experience.
- Experience in primary care setting.
- Strong clinical knowledge and/or qualification in respiratory conditions or willingness to develop in this area.
