- 6 sessions (flexible)
- 27 patients per day total
- 15 morning / 12 afternoon
- Flexible appointments: 10, 12 or 15 minutes, or a mixture of each
- GP led template - control over how your day is structured
- Admin built into the day with flexibility to complete remotely if needed
- Rare to no home visits - covered by dedicated paramedics and frailty team
- Shared on call system supported heavily by Advanced Paramedic Practitioner
- Mostly telephone based with strong clinical triage support
- Approximately 1 day per week
- Minimal enhanced access GP involvement (primarily covered by nurses and paramedics)
- Advanced Paramedic Practitioner + further Paramedic team
- Dedicated home visiting team, including frailty support
- Specialist Nurses and HCAs
- Full PCN MDT, including Social Prescribers, Care Coordinators, Digital Coordinators, Teenage Counsellor
