Responsibilities and Duties
- Assess, diagnose, plan, implement and evaluate treatment, interventions and care for patients presenting at the practice.
- Clinically examine and assess patient needs and plan clinical care accordingly.
- Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
- Prescribe and review medication in accordance with practice and national protocols.
- Undertake home visits.
- Undertake telephone advice calls.
- Maintain accurate record keeping with Systm1.
- Request and interpret tests appropriate to patients needs.
- Make referrals to other health professionals in primary and secondary care where appropriate.
Managing Self and Others
- Participate in an annual individual performance review.
- Manage own time, workload and resources effectively.
- Contribute to the effectiveness of the team by reflection on own and team activities and making suggestions on ways to improve and enhance the team's performance.
- Develop a leadership role within the practice.
- Undertake professional development as required by the role.
- Promote effective communication and relationship within the team.
- Participate positively and actively in clinical and practice meetings.
Qualifications
- Registered Professional.
- Evidence of advanced qualification.
Experience
- Minimum 1 year post registration experience.
- Experience in management of patients with complex and long term needs.
- Proven ability to evaluate safety and effectiveness of own clinical practice.
- Ability to work in a team.
- Excellent communication skills both written and verbal.
- Previous experience of working in a GP practice or primary care.

We are a small but growing team of 2 Partners, 3 Salaried GP's, 1 ACP, a nursing team of 4 and a management/admin team of 8.
