Responsibilities and Duties

• Assess, diagnose, plan, implement and evaluate treatment, interventions and care for patients presenting at the practice.
• Clinically examine and assess patient needs and plan clinical care accordingly.
• Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
• Prescribe and review medication in accordance with practice and national protocols.
• Undertake home visits.
• Undertake telephone advice calls.
• Maintain accurate record keeping with Systm1.
• Request and interpret tests appropriate to patients needs.
• Make referrals to other health professionals in primary and secondary care where appropriate.

Managing Self and Others

• Participate in an annual individual performance review.
• Manage own time, workload and resources effectively.
• Contribute to the effectiveness of the team by reflection on own and team activities and making suggestions on ways to improve and enhance the team's performance.
• Develop a leadership role within the practice.
• Undertake professional development as required by the role.
• Promote effective communication and relationship within the team.
• Participate positively and actively in clinical and practice meetings.

Qualifications

• Registered Professional.
• Evidence of advanced qualification.

Experience

• Minimum 1 year post registration experience.
• Experience in management of patients with complex and long term needs.
• Proven ability to evaluate safety and effectiveness of own clinical practice.
• Ability to work in a team.
• Excellent communication skills both written and verbal.
• Previous experience of working in a GP practice or primary care.
