We have an opportunity for a Clinical Pharmacist to join our team in the Pharmacy Department. This is a bank role working on an 'as and when required' basis; you will be required to supplement staffing levels during busy periods and provide cover for existing staff in times of annual leave or absence.

Duties of this role include:

  • Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation.
  • Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations.
  • Support safe prescribing through reconciliation, clinical screening, and dose adjustment.
  • Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements.
  • Promote safe and cost effective use of medicines by supporting adherence to local and national policies.
  • Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability and supply continuity.
  • Monitor and report medication incidents, near misses, and adverse drug reactions in line with hospital protocols and MHRA guidance.

Applicants should meet the following criteria:

  • A degree in Pharmacy (MPharm, BSc or BPharm).
  • GPhC registration as a Pharmacist.
  • Previous experience in a clinical hospital pharmacy role.
  • Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation.
  • Excellent communication and clinical decision making skills.
  • Ability to work autonomously and as part of a multidisciplinary team.
  • Confident using electronic prescribing and medication administration systems (e.g., EPMA).
