Main duties of the job:

  • Following a comprehensive induction programme, you will undertake a rotational clinical pharmacist role working closely with pharmacy colleagues and the wider multidisciplinary team.
  • You will engage directly with your patients to achieve medicines optimisation throughout their inpatient journey from medicines reconciliation through to discharge.
  • Rotations include a variety of specialities including biologics, frailty unit, paediatrics/neonatal, emergency department, healthcare of the older person, acute admissions unit and post operative surgery.
  • In addition to providing a pharmacy service at ward level, you will also undertake the role of responsible pharmacist when working in outpatient areas on a rotational basis as well as contribution to the on call services.
  • We will provide all the resources you need to help develop your knowledge and skills during each rotation, providing support from senior clinical pharmacists, rotation objectives and study time to complete diploma work.
  • We are continually looking to improve and develop our team and services, so all our clinical pharmacists are encouraged to contribute to our Clinical Improvement Group and Health and Wellbeing groups.

Detailed job description and main responsibilities:

  • Through a varied rotational programme, work within multidisciplinary healthcare teams to provide ward based clinical pharmacy services, taking responsibility for clinical screening, medicines reconciliation, and optimisation of prescriptions within own scope of competence.
  • Deliver medicines optimisation and the safe transfer of care around medicines, including supporting accurate medicines reconciliation at admission and discharge and effective communication with primary and secondary care providers.
  • Undertake a postgraduate diploma in clinical pharmacy, subject to funding being available.
  • Undertake necessary continuing professional development in line with General Pharmaceutical Council standards and practise in accordance with the GPhC standards of conduct, ethics and performance.
  • Provide clinical pharmacy support to inpatient and outpatient dispensaries and undertake the role of Responsible Pharmacist when required.
  • Participate in the provision of medicines information and advisory services to patients and their relatives, medical and nursing staff, and other healthcare professionals and organisations, including patient counselling and support to enable safe and effective use of medicines.
  • Actively contribute to medicines safety by identifying, reporting, and learning from medication incidents, near misses, and risks, in line with Trust medicines governance and incident reporting systems.
  • Provide professional supervision to pharmacy technical and support staff.
  • Contribute to the training and development of trainee and undergraduate pharmacists and pharmacy technicians.
  • Participate in clinical audit and quality improvement initiatives aimed at assuring and improving the quality, safety, and effectiveness of prescribing and pharmacy practice.
  • Contribute to the practice and development of quality governance throughout the pharmacy department.
  • Undertake identified clinical and professional training and actively participate in the staff appraisal process.
  • Contribute to the delivery and development of clinical pharmacy services across the Trust.
  • Provide training and education to other healthcare professionals, supporting safe and effective medicines use.
  • Utilise electronic prescribing, medicines administration, and other digital systems effectively to support safe, accurate, and auditable pharmacy practice.

General responsibilities:

  • Undertake any training (managerial and professional) identified through the staff appraisal process.
  • Participate in on call duties, weekend and Bank Holiday working, early and late shifts, in accordance with agreed rotas and service needs.
  • In the event of a disruptive incident, all Trust employees may be required to attend work if they are fit, well, and able to do so, in line with business continuity and major incident plans.
  • Maintain confidentiality at all times relating to patients, staff, and Trust information, in accordance with information governance and data protection requirements.
  • Actively participate in the Staff Appraisal Process.
  • Be familiar with and comply with relevant operational, personnel, health and safety, and Trust policies and procedures, including Fire, COSHH, No Smoking and Alcohol policies.
  • Maintain a safe working environment by adhering to local infection control policies and promoting safe professional practice.
  • Contribute to an open and learning culture by supporting the Duty of Candour and raising concerns where patient safety, professional standards, or wellbeing may be compromised.

Person specification:

  • Qualifications: Registered with the GPhC, Membership with other relevant professional bodies e.g. UKCPA, RPS
  • Experience: Insight into the practice of hospital based clinical pharmacy, Experience of hospital practice at pre registration level
  • Skills and Knowledge: Good grasp of the principles of clinical pharmacy / therapeutics, Knowledge of hospital pharmacy practice, Knowledge of modern approaches to Medicines Management / optimisation, Evidence of CPD, Extended knowledge of hospital pharmacy practice and Medicines Management / optimisation
  • Personal Attributes: Energetic and enthusiastic, Strong team skills, Self directed and able to work independently, Capable of undertaking Post Graduate study, Ability to prioritise and work under pressure
