The closing date is 04 June 2026. Following the release of ARRS GP information and funding, we are looking for an enthusiastic, forward-thinking GP to join Healthier West Wirral Primary Care Network. We will consider applications for between 8 and 9 sessions with a start of August 2026.

Main duties of the job

  • Undertake home visits for patients in need of medical attention, including those who are housebound or unable to attend surgery and have been referred by their practice.
  • Provide comprehensive and timely care to patients, ensuring appropriate diagnosis, treatment, and follow up.
  • Provide general practice care across the 5 GP Practices within Healthier West Wirral PCN, including consultations, diagnosis, treatment, and management of both acute and chronic conditions.
  • Work collaboratively with practice teams, including nursing staff, healthcare assistants, pharmacists, and other ARRS roles.
  • Work within a multidisciplinary team, gaining exposure to innovative primary care models such as AVS.
  • Contribute to improving access to care for patients, reducing waiting times, and supporting clinical capacity within the practices.

Healthier West Wirral PCN supports a population of over 70,000 patients across 5 different practices including Marine Lake Medical Practice, Myrtle Group Practice, Upton Group Practice, West Wirral Group Practice and Greasby Group Practice. We are an ambitious and forward-thinking organisation that has already built a large PCN team including Physician Associates, ACPs, Pharmacists, Pharmacy Tech, Nurses, Care Coordinators and Paramedics.

Job Responsibilities – Acute Visiting Service (AVS) 4 Sessions

  • Undertake home visits for patients in need of medical attention, including those who are housebound or unable to attend surgery who have been referred by GPs.
  • Provide comprehensive and timely care to patients, ensuring appropriate diagnosis, treatment, and follow up.
  • Work alongside physician associates and paramedics already established in the Acute Home Visiting Team.
  • Work closely with practice staff, community health teams, and the wider PCN to ensure continuity of care for patients across services.
  • Ensure accurate and timely documentation of visits, clinical assessments, and treatment plans.
  • Ensure there is appropriate feedback to referring GP/clinician as required.
  • Triage and treat patients wishing to see a health care professional, making any necessary referrals to other members of the primary health care team.
  • Manage patients with problems needing referral to secondary care.
  • Ensure service is safe and effective and remains within boundaries of competence, and to acknowledge limitations.
  • Advise patients on general healthcare and minor ailments, with referral to other members of the primary and secondary health care team as necessary.
  • Work from the hub and within communities as an autonomous practitioner caring for patients and families.
  • Ensure that personal and professional clinical standards are maintained.
  • To communicate at all levels within the team ensuring an effective service is delivered.
  • To maintain accurate, contemporaneous health-care records appropriate to the consultation.
  • Ensure evidence-based care is delivered at the highest standards ensuring delivery of high quality patient care.
  • Works with local policies and procedures.
  • To achieve and demonstrate agreed standards of personal and professional development in order to meet the needs of the service.
  • To participate in the audit process, evaluation and implementing plans and practice change in order to meet patient need.

Job Responsibilities – GP Practice Sessions 3 Sessions

  • Provide general practice care, including consultations, diagnosis, treatment, and management of both acute and chronic conditions.
  • Face to face and telephone consultations.
  • Checking and signing repeat prescriptions.
  • Work collaboratively with practice teams, including nursing staff, healthcare assistants, pharmacists, and other ARRS roles.
  • Administration – dealing with queries, patient test results, referral letters NHS/private, task, emails and other paperwork and correspondence in a timely manner.
  • Support the management of long-term conditions and deliver preventative care.
  • Contribute to improving access to care for patients, reducing waiting times, and supporting clinical capacity within the practices.
  • Cover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate.
  • Participate in clinical meetings, multidisciplinary team meetings, and other practice/PCN level discussions to improve patient care.
  • Reports to clinical director.

There is a need to travel between sites, therefore ability to drive and use own car is essential. Applicants of all backgrounds are encouraged to apply.

Person Specification

  • Be included in NHS England's list of medical practitioners as per section 91 of the NHS Act 2006.
  • Hold full registration with the General Medical Council (GMC) on the General Practitioner Register.
  • Be a Newly Qualified GP who obtained their Certificate of Completion of Training (CCT) in General Practice within the last two years.
  • Not be prohibited from providing services due to regulation breaches (e.g., NHS Performers Lists Regulations).
  • Not be suspended from practice by the NHS or from the GMC's list of medical practitioners.
  • Not be subject to any interim suspension orders from the GMC under section 41A of the Medical Act 1983.
  • Be employed on terms at least as favourable as those outlined in the Model terms and conditions of service for a salaried general practitioner employed by a GMS practice.
  • Cannot be reimbursed under this scheme if previously substantively employed as a GP in general practice, unless explicitly agreed upon with the commissioner.
  • May be employed in temporary capacities, such as locum doctors, but this must be explicitly stated and agreed upon with the commissioner.
  • Have the legal right to work in the UK.
  • Hold appropriate medical indemnity insurance covering all professional duties.
  • Complete an enhanced Disclosure and Barring Service (DBS) check.
  • Possess a valid digital badge for secure system access and patient care delivery.
  • Be willing to engage in ongoing professional development as required by the ARRS.
