An experienced Advanced Clinical Practitioner is required to join the award winning Intensive Home Support Service (IHSS), delivering holistic, comprehensive assessments and care for patients in their home environment. The role involves advanced practice, clinical leadership, and collaboration with multi professional teams across East Lancashire.

Main duties:

  • Apply advanced clinical practice knowledge to assess and treat patients admitted to IHSS, using physical examination, diagnostic testing, and advanced health needs assessment.
  • Provide independent clinical line management and professional leadership across the East Lancashire footprint, supporting the development and extension of advanced practice skills.
  • Offer clinical oversight and input to IHSS caseload patients, and participate in collaborative projects with associated emergency departments and ambulance services.
  • Work across extended hours and a 7 day schedule (excluding night duty).
  • Assist in reducing avoidable hospital conveyances and deliver a 2 hour Urgent Community Response service.

Person specification:

Qualifications:

  • Post graduate qualification in a relevant speciality or equivalent experience.
  • MSc in Advanced Clinical Practice or equivalent.
  • Recognised teaching or mentorship qualification at degree level or equivalent.
  • Leadership module or equivalent leadership study experience.

Experience:

  • High level clinical experience in a range of settings.
  • Proven experience managing complex cases.
  • Evidence of supervising and mentoring team members.
  • Experience in partnership and collaborative practice.
  • Participation in research, audit, and evaluation activities.
  • Conducting appraisals and developing personal development plans.
  • Leading change and innovation initiatives.

Knowledge:

  • Clinical knowledge inclusive of current evidence based practice.
  • Competence in patient assessment and case management plan development.
  • Ability to interpret diagnostic tests and adjust interventions.
  • Knowledge of chronic disease management and health system integration.
  • Understanding of governance, service outcomes, and advanced practitioner roles.
  • Excellent written and verbal communication, presentation, and facilitation skills.
  • Strong organisational, time management, and analytical skills.
  • Proficient with information technology and internet based communications.

  • Ability to cover a rota of 7 days per week, 365 days per year.
  • Fitness to carry out duties with reasonable adjustments as required.
  • Current driving licence.
