This is an opportunity for a qualified Social Worker, Occupational Therapist or Nurse
to join our established and experienced multi disciplinary team. You will be working as
a Hospital Discharge Coordinator covering the two Older Adult Mental Health teams
in Monmouthshire, providing assessment and care planning, including a specialist
Mental Health and Care and Treatment Planning service to meet the presenting needs
of individuals and their carers.

SALARY - IMPORTANT INFORMATION: Salary would be pro rata if not working full time hours or 52 weeks per year.
i.e. 37 hours per week / 32.5 per week for School Teachers
Temporary End Date (if applicable):
Location: Maindiff Court, NP7 8NF, United Kingdom. This may change in the future if the service location needs to relocate. Relocation or disturbance expenses will not be paid if this happens.
