Jewish Care is seeking a dedicated and highly organised Clinical Care Coordinator to support the Registered Manager and Care Management Teams across our Mental Health Residential Care Homes. This pivotal role supports the effective running of Sidney Corob House, providing care for adults aged 50+ with mental health needs, and Jack Gardner House, a safe and welcoming home for Jewish adults aged 18+ with mental health needs.
You will play a key role in coordinating clinical and care activity, supporting compliance, driving quality improvement, and ensuring residents receive high-quality, person-centred care. The ideal candidate will have strong organisational skills, a solid background in mental health care, and the ability to work collaboratively within a multidisciplinary environment.
This post is working 39 hours per week (Monday to Friday) 8am to 4pm.
What You Will Be Doing- Supporting the Registered Manager & Care Management Team
- Supporting day-to-day operational management of both care homes
- Providing administrative and logistical support to ensure smooth running
- Contributing to strategic planning and implementation of policies and procedures
- Resident Care Coordination
- Undertaking comprehensive clinical assessments of residents’ mental health needs
- Coordinating and monitoring the delivery of clinical and care services
- Team Collaboration & Communication
- Facilitating effective communication between managers, staff, residents, and families
- Supporting and attending clinical reviews and meetings
- Acting as a liaison with healthcare professionals and community services
- Compliance & Quality Assurance
- Ensuring care delivery meets regulatory, policy, and best practice standards
- Maintaining accurate documentation, audits, and records
- Participating in audits and quality improvement initiatives
- Staff Training & Development
- Supporting training and development of staff in mental health care practices
- Promoting continuous learning and professional development
- Resident Advocacy & Support
- Advocating for residents’ rights and wellbeing
- Providing emotional support to residents and families
- Supporting residents in accessing appropriate healthcare services
- Essential Requirements
- NVQ/QCF Level 3, 4 or 5
- RMN or RGN qualification or equivalent mental health qualification
- Experience working with older people in a care or nursing home environment
- Proven experience in day-to-day management of a care or nursing home
- Experience of leading and supervising teams
- Experience of medication administration and emergency care
- Experience of security and building management
- Experience working with people living with dementia
- Excellent understanding of social care regulations
- Strong knowledge of dementia care, end-of-life care, capacity, Safeguarding and DoLS
- Commitment to person-centred care and the rights of older people and individuals with physical, sensory, and cognitive impairments
