Main duties of the job
- Assist the whole practice team to helpdeliver effective implementation of management of patients and theirneeds.
- Managing the nursing care of patientswith chronic disease, employing recall systems suitable to the practiceneeds. This includes using the practice computer systems for input ofinformation for recall and screening purposes. Accurate and efficientinput of data for contract purposes is also required
- Contribute to the development of policies/ protocols / guidelines within the practice.
- Adhere to infection control measureswithin the practice according to local and national guidelines.
- Aid the practice team with implementationof new initiatives in the delivery of chronic disease management services.
- Encourage the development of integratedservices within Primary Care to include integrated practice based teamsand multidisciplinary working.
- Recognise the opportunities for workingin partnership with the Primary Health care team.
