The predicted salary is between 36000 - 60000 Β£ per year.

Do you want to join a fast-paced team where no two days are the same? Stockport older people's community mental health team are looking for motivated band 6 nurses. The team works off a shared caseload, which has been a very successful change within the team, improving risk management and reported to be less stressful than the previous CMHT model. Stockport older people's CMHT works with the single point of entry team. The SPOE team completes the urgent referrals and CMHT completes the Crisis intervention. The team is dynamic, passionate, and problem-solving is key. We work closely with the wards to facilitate discharge. We have regular case formulation sessions and often provide bitesize training opportunities. We have groups for service users and a monthly carers group.

The team is made up of Band 4, 5, and 6 Nurses, Band 3 Support Workers, Band 6 Occupational Therapists, Psychology, and we also work very closely with the medics. The team works 9-5 and is a 7-day service, with weekends rotated fairly. Every day a different team member is allocated to be the duty/gatekeeper worker.

The post-holder will ensure the highest standard of nursing care is provided in accordance with the policies and procedures of Pennine Care NHS Foundation Trust. The practitioner will provide interventions to service users within the CMHT and will work with the multidisciplinary team to offer a team approach to supporting service users in the community or for those on the discharge pathway from inpatient settings.

Responsibilities include:

  • Contributing to case formulation and treatment plans.
  • Contributing to service development in collaboration with the wider team.
  • Participating in the education programmes of nursing students and other disciplines.

Qualifications and Experience:

  • Professional qualification in Mental Health Nursing
  • Registration with HCPC / NMC
  • Experience in community mental health work
  • Willingness to undertake further training relevant to the practitioner role resulting from legislative or service changes, as appropriate.
  • Experience of supervision
  • Experience of some crisis work/interventions
  • Knowledge of CPA policy
  • Experience in working with people with moderate to severe mental health needs
  • Extensive knowledge/experience in social inclusion and recovery model of care
  • Knowledge of mental health legislation
  • Experience of MDT working
  • Ability to care coordinate and be responsible for a caseload
  • Ability to work effectively as part of a MDT
  • Ability to supervise junior staff/students
  • Effective time management
