Thisis a dual HCA role involving working in an organised and structured wayalongside the practice home visiting team out in the community as well asundertaking HCA clinics in Practice.

Thisis a vital role involves supporting the home visiting team with conducting homevisits to undertake health care related activities i.e. phlebotomy, bloodpressure, height, weight, lifestyle areas such as smoking and alcohol, ECG,chronic disease annual reviews.

Youwill also be required to work in practice working under the direct supervisionof the SeniorNurse Lead and strictly in accordance with specific practice guidelinesand protocols, the Healthcare Assistant will assist the practice clinical teamin the provision and delivery of prescribed programmes of patient care. Theywill work collaboratively with the general practice team to meet the needs ofpatients, following policy and procedures.

To assist in the delivery of high-quality,holistic care to patients within a primary care setting, supporting the nursingand medical teams in promoting health, preventing illness, and managinglong-term conditions.

Job responsibilities

Dutieswill include, but are not limited to:

Support the Practice home visitingservice seeing patients in their own homes

Injectables (supported by theory,competencies & certificate & in-line with national search criteria asfoundation of PSD):

COVID Injections - Following provisionof Certificate & Competencies documentation

Influenza vaccinations (Adults 18 yearsand over) Following provision of Certificate & Competencies documentation

B12 injections - (Following initiationby GP & loading dose by Practice Nurse) - Following provision ofCertificate & Competencies documentation

Pneumonia vaccinations - (Adults over65) - Following provision of Certificate & Competencies documentation

Shingles vaccinations - (In line withNational searches) - Following provision of Certificate & Competenciesdocumentation

NHS Health Checks

Quality Outcomes Framework long termcondition management - working alongside Practice Nurse for patient reviews,any areas of QOF appropriate to role and at discretion of Practice Nurse, GP& Practice Manager

Fitting ambulatory blood pressuremachines

Fitting ambulatory ECG monitors

Urinalysis (Dip stick urines in linewith our urines policy liaising with Practice Nurse/GP)

Weight Management checks (where feltappropriate by GP/Practice Nurse/Dietician)

Diabetes foot checks as part of annualdiabetes reviews

Simple dressings (supported by PracticeNurse)

ECG recording

Processing and management of laboratorysamples requested by GPs/nurses

Vaccine/cold chain storage, monitoringand recording

Surgical equipment and vaccinere-stocking and stock rotation

Clearing and re-stocking consultingrooms

Preparing and maintaining environmentsand equipment before, during and after patient care interventions, includingassisting GPs during the performance of minor operations/joint injectionswhere appropriate

Assisting in the assessment andsurveillance of patients health and well-being

Undertaking specific clinical activitiesfor named patients that have been delegated and taught specifically inrelation to that individual

Helping to raise awareness of health andwell-being and how it can be promoted

Assisting with the collection andcollation of data on needs related to health and wellbeing

Be expected to see patients in their ownhome as and when required

Any other clinical or administrativeduties in line with your individual skill set and felt appropriate to thisrole
