We are delighted to open applications for 2 part time Patient Coordinators (RVN) to join our team at Hamilton Specialist Referrals on a 6 month fixed-term contract basis. Our successful candidates will work collaboratively within our clinical nursing team, as well as acting as a point of contact for our clients.
Coordinating a Seamless Client Experience
Join our nursing team at Hamilton Specialist Referrals as a Patient Coordinator, where you’ll play a crucial role in delivering exceptional client service and seamless patient care coordination. From admission to discharge, you’ll be the central liaison, ensuring clear, compassionate communication with clients and working closely with clinicians and the Clinical Administration team. Clinically, you will be based predominantly within our Wards team, providing high standards of care to a range of complex referral cases.
Key Responsibilities
- Client Communication: Provide a consistent and client‑focused experience by keeping clients informed throughout their pet’s journey, addressing questions, providing guidance on care plans.
- Patient Care: Oversee patient admissions and discharges, and manage inpatient care coordination whilst working clinically as a key part of our wards team. This will include I/V catheter placement, blood sampling, TPR & BP assessment, sedation/recoveries and nurse consults.
- Team Collaboration: Act as a pivotal link between clients, clinicians, and the Clinical Administration team to maintain clear and effective communication, supporting the best possible patient outcomes.
Supporting you in your new role
- Experience within a referral setting is advantageous but not essential.
- We are also proud to offer central support via IVC Evidensia’s \”Introduction to Referral Nursing\” training modules, specifically designed for RVNs moving from first opinion to referral.
- To be successful you will be a proactive, enthusiastic and approachable person who is not only a great team player, but can also use their own initiative and self‑motivate.
- Excellent communication and interpersonal skills, and experience of client liaison.
- You must be a qualified RVN with current RCVS registration.

We have a dedicated Imaging suite, which includes a 16‑slice CT scanner, 1.5 Tesla MRI, fluoroscopy, separate radiography and ultrasound room.

Working Pattern
This is a part‑time position on a fixed term contract basis. The role is 22hrs/week over 2 days, Monday & Friday each week (07:30‑19:30) with occasional flexibility required in line with staffing and business levels.
